All forms are fillable (with a PC only-not a Mac, sorry) so you can save them, fill them out, send them via email

If you have a Mac, you can either 1) print them out, fill them in, and scan them (or bring them in with you for your appointment).

2) come 15 minutes early to your first appointment.
